1. Multiple Wastewater Streams
Wastewater can originate from wards, laboratories, dialysis, CSSD, pathology, operation theatres, laundry, kitchens, canteens and staff facilities. These streams differ in pH, organic load, suspended solids, detergents, disinfectants and potential clinical contaminants. Improper mixing can make treatment more difficult and may expose a conventional STP to loads it was not designed to handle. Stream identification, segregation and collection planning are critical.
2. Organic Load, Pathogens and Healthcare Contaminants
Hospital wastewater may contain blood and body-fluid residues, organic matter, laboratory chemicals, detergents, pharmaceutical residues and microorganisms. Variable BOD/COD, suspended solids and pathogen levels require adequate biological treatment, filtration and validated disinfection. Treatment should therefore go beyond simple settling and include polishing and disinfection appropriate to the final discharge or reuse requirement.

4. Variable Occupancy and Hydraulic Load

Flow and load variations of approximately 30–60% may occur in some facilities, depending on actual operating patterns; this is a design consideration, not a universal fixed value. An STP designed around only average flow may become unstable during peaks. Equalization, realistic peak hydraulic calculations and biological process resilience are essential.

5. STP and ETP Segregation

Where dedicated treatment is required, separate collection, equalization and appropriate pre-treatment prevent concentrated chemical or clinical streams from disturbing the domestic STP. Discharge routing should follow wastewater characterization and applicable requirements.
6. Space Constraints in Operational Hospitals
Hospitals often place utility systems in basements or restricted service areas. Retrofitting a wastewater plant may require compact equipment, low-headroom solutions and carefully planned maintenance access without interrupting hospital operations.
